Insurance and Billing

Our physicians participate with most of the health plans in the area and we are happy to file claims for you regardless of our network participation.  Below is a list of the carriers we are contracted with.  If you are unsure about our participation with your plan, please contact your insurance carrier for the most accurate information. If your plan requires referrals from your primary care physician, it is your responsibility to obtain that referral.  If office charges are denied for lack of a referral, you will be responsible for those charges. We will provide you with care that adheres to your insurance guidelines, however, we must have your help.  It is impossible to keep track of all individual plan requirements.  We depend on you to know your coverage requirements.  We will file your claims for you and will obtain prior authorization for surgeries and diagnostic procedures, but you are ultimately responsible for knowing your benefits and keeping your account current. Co-payments are a contract between you and your insurance carrier.  They must be paid at the time of each visit or your appointment will be rescheduled.  Please have your insurance cards with you at each appointment and notify us immediately of any changes. Your insurance is a method of assisting you with your medical expenses and is NOT a substitute for payment.  You are responsible for any applicable co-pays, coinsurance, deductible, and non-covered services.  We accept cash, check, Visa, and Mastercard.  Depending on the balance due, interest-free payment plans may be available for accounts in good standing. We DO NOT accept third party liability claims.  If your injury is part of a liability claim, you are responsible for payment at the time of service. For uninsured patients, we require an initial payment of $150-300 that is due when you check in for your first appointment.  This may or may not cover the full charge for your visit.  Payment is due in full for all subsequent visits at the times services are rendered.
